Keep a transfer between organizations from becoming the moment an animal's story gets rebuilt from scratch.

A transport handoff is still a handoff. This template keeps the source, the point person, and the open questions attached.

What is transferring

  • Name the animal, case reference, and both organizations.
  • Attach intake, medical, and behavior source notes.
  • Note what is confirmed versus still unknown.
